Position: Family Team Meeting Facilitator for Community Collaborations for Children Program

Reports To: Head Start Director

Duties:

  • Attend the data report training.
  • Facilitate in the best interest of the child.
  • Approach the family from a position of respect and collaboration
  • Establish a referral process, include a plan to reach out to community partners.
  • Referrals must include information about all safety risks for the child(ren), the family, and staff.
  • Contact and schedule meetings within seven working days of referral.  Arrange time and meeting locations, and notify all participants of the meeting in writing, by telephone or face to face. 
  • Identify the strengths of the family.
  • Provide neutral facilitation to ensure an objective discussion of issues and services, among other things.
  • Engage the family around concerns for child safety. 
  • Help the family achieve a clear understanding of risk issues.
  • Develop an action plan with the family that includes timelines for meeting goals.
  • Facilitate discussion, develop tasks and objectives, and ensure that all parties understand their role.  Information included in an action or case-plan should reflect goals for the family.
  • Promote parent (caregiver) and child attachment
  • Use preventive measures or techniques to redirect any potential for crisis and/or conflict to occur in an effort to enhance positive interaction.
  • Develop and maintain a written plan for each case for uninvited individuals who attempt to disrupt the FTM.  Keep written plan on file and ensure that family, workers, and referral source have a copy. 
  • Submit follow-up documentation to the referring agency within seven (7) calendar days of the completion of the FTM.  Documentation should include an action plan, FTM summary, and/or other notes taken during the meeting.
  • Notify the referring agency in writing of any issues within the family that arise before or after a FTM. 
  • Submit a Regional Quarterly Report to CCC Technical Assistants and the Division of Administration and Finance Management.
  • Submit a Quarterly Data Report to the CCC Technical Assistants unless informed otherwise.
  • Attend yearly Peer Exchange
  • All documentation and discussion with the family must be confidential.
  • Collect client satisfaction surveys for each family and maintain on file. 
  • All duties deemed necessary.

Education Requirements:

  • Completion of  at least 30 college hours and have one year of experience providing similar services OR
  • Two years providing similar services.

Required Training upon Hire:

  • the Family Wellness Curriculum training
  • the 3-day Foundational Training Part 1 and Part 2 at least twice.
  • the one-day Part 1 and Part 2 FTM service specific training at least twice.
  • IF staff  have completed Foundational Training Part 1 and Part 2 at least twice then staff have an option to:
    • Attend 15 hours per year of pre-approved service related training (this training must be approved by the CCC Frankfort Technical Assistant and must be completed  each year OR
    • Staff may choose to attend Foundational Training Part 1 and/or Part 2 more than twice.
  • IF staff have completed the one-day FTM service training at least twice then staff have an option to:
    • Attend an additional 3 hours per year of pre-approved service related training (this training must be approved by the CCC Frankfort Technical Assistant and must be completed  each year OR
    • Staff may choose to attend the one-day FTM service specific training more than 2 times.
